Topic
Emerging and Regenerative Therapies in Androgenetic Alopecia: From Current Evidence to Innovation
Program description
The medical treatment of androgenetic alopecia (AGA) is changing faster than ever. Classic therapies (minoxidil and 5-alpha reductase inhibitors) are being joined by new topical and oral antiandrogen drugs, autologous regenerative therapies, extracellular vesicle-derived products, and new ways of administering drugs to the scalp. At the same time, cellular research opens the door to strategies that aim to make the follicle insensitive to dihydrotestosterone (DHT).
This growing offering often reaches the clinic before the evidence. Patients ask about exosomes, PRP, or ‘new molecules’ they saw on social media, and the hair restoration surgeon needs clear criteria to decide what to offer, to whom, how, and with what expectations. This webinar brings together speakers with clinical and research experience to critically and practically review the current state of these therapies and their place within the comprehensive management of patients with AGA, before and after hair transplantation.
Description of Educational Need
This 2.5-hour webinar, “Emerging and Regenerative Therapies in Androgenetic Alopecia: From Current Evidence to Innovation,” addresses a specific gap: the distance between what is offered in the market and what is supported by evidence. Many physicians incorporate PRP, exosomes, autologous micrografts, or new administration techniques without standardized protocols, without knowing their regulatory status, and without an objective way to measure results.
The program is designed to offer an updated and critical review, organized into two blocks:
- Pharmacological therapies and translational research: emerging drugs for AGA and dermal papilla chimerization as an experimental strategy to confer DHT resistance.
- Regenerative therapies and drug delivery: PRP, exosomes, Rigenera (autologous micrografts by mechanical disaggregation), and CRTgro, an intradermal drug delivery technique.
For each therapy, the mechanism of action, the quality of available evidence, protocols, safety, regulatory status, and integration with hair transplantation will be reviewed. The session will conclude with a discussion among all speakers on how to combine these tools ethically and evidence-based, and how to communicate realistic expectations to patients.
Learning Objectives
By the end of the webinar, participants will be able to:
Pharmacological Therapies and Translational Research
- Emerging drugs in AGA
- Identify new molecules and therapeutic pathways under development or recently incorporated (e.g., topical antiandrogens such as clascoterone and pirilutamide, topical finasteride, low-dose oral minoxidil, and intradermal dutasteride).
- Compare their mechanism of action, efficacy, safety profile, and development phase against conventional therapies.
- Select the most appropriate therapy according to the patient’s profile (sex, age, comorbidities, reproductive desire, tolerance to adverse effects).
- Dermal papilla chimerization
- Understand the role of the dermal papilla and the androgen receptor in DHT-induced follicular miniaturization.
- Describe the concept of dermal papilla chimerization as an experimental strategy to confer DHT resistance in susceptible follicles.
- Recognize the current state of research, its limitations, and the challenges of bringing it to clinical practice.
Regenerative Therapies and Drug Delivery
- Platelet-rich plasma (PRP)
- Analyze current evidence on PRP in AGA and the variables that explain the heterogeneity of results (preparation system, platelet concentration, activation, frequency, and application technique).
- Apply patient selection criteria and an evidence-based treatment and maintenance protocol.
- Exosomes
- Explain what extracellular vesicles and products marketed as ‘exosomes’ are, and their proposed mechanism in the follicular cycle.
- Critically evaluate available clinical evidence, risks, variability among products, and their regulatory status.
- Communicate realistic expectations to patients and obtain adequate informed consent.
- Rigenera
- Describe the rationale for autologous micrografts obtained by mechanical disaggregation and the procedure technique.
- Review published evidence on its efficacy in AGA and its indications as a standalone therapy or complementary to hair transplantation.
- [Topic to be determined] – Dr. Giselle Rodríguez
- [Pending objectives]
- CRTgro
- Describe the intradermal drug delivery technique using a dermatographic microneedling device and its differences from conventional mesotherapy.
- Understand the protocols, mixtures used, technical parameters, and management of adverse effects.
- Integrate the technique into a combined treatment plan and the pre- and postoperative management of hair transplantation.
Clinical Integration
- Apply a decision-making model that combines medical therapy, regenerative therapies, and surgery, adapted to each patient and supported by evidence and objective measurement of results (trichoscopy and standardized photography).
