Program Description
Hair transplantation has become a popular option for transgender individuals seeking gender affirmation. However, unlike traditional hair transplant patients, transgender individuals require a multidisciplinary approach that includes collaboration among hair restoration surgeons, endocrinologists, psychiatrists, plastic surgeons, and other specialists. A comprehensive clinical evaluation and careful surgical patient selection are essential before proceeding with hair transplant surgery.
Educational Needs Description
This 2.5-hour webinar will focus on “Hair Transplantation in Transgender Men and Women: Integrating Surgical, Medical, and Psychological Care.” Hair transplantation is one of the surgical procedures that may be used as part of gender-affirming care to enhance appearance, improve self-esteem, and support patients’ self-image.
The webinar is designed to provide an important foundational review of the principles, challenges, and evolving strategies in hair restoration surgery for transgender patients as an integral part of feminization or masculinization. The session will address the comprehensive, multidisciplinary management of transgender patients, including psychological care and hormone therapy. Hair transplantation may be incorporated into a broader surgical treatment plan alongside various plastic, gynecologic, urologic, or other procedures, or it may be performed as a standalone surgical treatment, depending on each patient’s individual goals and needs.
Feminization and masculinization may include procedures intended to enhance facial harmony, including modification or restoration of the hairline, eyebrows, mustache, beard, and/or sideburns, depending on whether the patient is undergoing male-to-female or female-to-male gender affirmation. Psychological evaluation and support are important components of the multidisciplinary approach and may contribute to determining patient suitability for hair transplantation and developing an individualized surgical plan.
The WPATH (World Professional Association for Transgender Health) guidelines and the informed consent model are two important frameworks used in evaluating patients for gender-affirming surgical procedures. In addition, faculty will present a series of relevant clinical cases involving feminization and masculinization, providing perspectives from their respective areas of expertise.
Learning Objectives
At the conclusion of this webinar, participants will be able to:
Feminine Transition (Transgender Women)
- Recognize the importance of an initial evaluation by a psychologist/psychiatrist and/or gender care team, as appropriate to the country, including baseline hormone testing and a general health assessment (liver and kidney function, coagulation, and blood pressure).
- Evaluate hormone therapy, including:
- Antiandrogens: spironolactone, cyproterone acetate, or GnRH analogues to reduce testosterone levels.
- Estrogens: oral, transdermal patches, or injectable formulations.
- Recognize which changes are not reversed by hormone therapy and the role of plastic surgery in patient feminization.
- Identify the anatomical and reconstructive approach to hair transplantation as a surgical option, either as a standalone procedure or in combination with other surgeries, according to each patient’s individual goals.
- Understand the anatomical principles that contribute to the perception of femininity in the upper third of the face.
- Assess patient candidacy for hair transplantation or feminization of the hairline and identify different strategies for hairline design based on each patient’s individual anatomy, facial analysis, expectations, and donor-area limitations.
- Describe the clinical criteria that determine the indication and appropriate timing for hair transplantation and feminization procedures, as well as relevant contraindications.
- Apply a decision-making model that integrates patient selection, medical therapy, and hair restoration as part of the overall gender-affirming process.
Masculine Transition (Transgender Men)
- Recognize the importance of a comprehensive initial evaluation, including appropriate psychological and medical assessment and baseline laboratory testing.
- Recognize the role of hormone therapy:
- Testosterone: injectable formulations (the most common), gels, or patches, including its progressive effects, changes that may become visible within the first few months, and irreversible changes.
- Assess patient candidacy for hair restoration procedures involving the eyebrows, mustache, beard, sideburns, chest, and/or hairline as part of achieving facial and body harmony in female-to-male transition, and recognize the important role of plastic surgery in masculinization.
